Start by Gathering and Preparing Your Documents

Fill out the online DS-11 application online, but DO NOT SIGN until directed to do so.

Provide Proof of Citizenship

You need to supply one of the following:

Previous U.S. Passport if you are over 15 years old
Certified Birth Certificate from the state, county, or city
Certificate of Citizenship or FS-240, DS-1350

Supply 2 copies of one of the following:

Travel ticket to a foreign country
Travel itinerary or e-ticket with reservation confirmation

Passport Photographs

Obtain 2 passport photos from a professional

U.S. Government Fees

Make a check or money order payable to “U.S. Department of State” for $190 (processing fee for an expedited passport). The check should have a pre-printed name and address. Temporary or starter checks are not acceptable.

Authorization Letter
