Make an appointment with your local Post Office, they will act as the accepting agent for the Passport Agency. Bring the following to your appointment:
- Birth Certificate or Certificate of Naturalization – for proof of citizenship
- 2 passport photos – 2”x2” White background
- Government issued ID – i.e. Driver’s license (original and a photocopy)
- Letter of Authorization (LOA) – Click Here – Please complete and sign 2 copies of LOA attached – Sign in Blue ink
- Completed but unsigned DS-11 Form– use this link https://pptform.state.gov/ – Complete online, print but Do Not Sign – The PO Agent will have you swear the oath and sign the DS-11. Collect the fees $209.53 Passport Agency fee and the Post Office fee of $35.00.
The Post Office agent will seal the (signed DS-11, signed LOA, photo, birth certificate, copy of your ID and fees) in an envelope and return the sealed envelope to you.
To proceed we will need the following sent to our office:
- Sealed envelope – Seal must only be broken by the Passport Agency
- Extra unsigned DS-11
- The extra passport photo
- Extra LOA – One will go into the sealed envelope and the other included with the material being sent to us.

Processing will be 5-7 business days from the date of submission to the Passport Agency. Your passport will be shipped directly to the address listed on your DS-82 Form.

- Government-issued ID of both parents i.e. Driver’s license (original and a photocopy)
- Letter of Authorization (LOA) – Click Here – Please complete and sign 2 copies of LOA Signed by both parents in Blue ink
- Completed but unsigned DS-11 Form– use this link https://pptform.state.gov/
- The PO Agent will have you swear the oath and sign by both parents the DS-11 Form. Collect the fees $179.53 Passport Agency fee and the Post Office fee of $35.00
The Post Office agent will seal the signed (DS-11, signed LOA, photo, birth certificate, copy of your ID and fees) in an envelope and return the sealed envelope to you.
