- 2 passport photos – 2”x2” White background
- Government-issued ID of both parents i.e. Driver’s license (original and a photocopy)
- Letter of Authorization (LOA) – Click Here – Please complete and sign 2 copies of LOA Signed by both parents in Blue ink
- Completed but unsigned DS-11 Form– use this link https://pptform.state.gov/
- The PO Agent will have you swear the oath and sign by both parents the DS-11 Form. Collect the fees $182.05 Passport Agency fee and the Post Office fee of $35.00
The Post Office agent will seal the signed (DS-11, signed LOA, photo, birth certificate, copy of your ID and fees) in an envelope and return the sealed envelope to you.

Required Documents:
- A completed US Passport Application (available on-site).
- Two (2) identical, 2″ x 2″ recent, professional, face-front photographs with a white or light background (no school photos accepted).
- An original or certified copy of a birth certificate with a raised seal and file date issued by a State or Municipal Bureau of Vital Statistics, including both parents’ names (no photocopies accepted), OR
- A Naturalization Certificate (no photocopies accepted), OR
- A previous United States passport
- A valid driver’s license, government identification, Green Card, valid passport from parent’s country, or a previous US passport issued after the applicant’s 16th birthday and not more than 15 years old
If the applicant cannot furnish the above identification, they can present:

- Three additional forms of identification (bring whatever you have) AND
- An identifying witness who has known the applicant for at least two years, is a US citizen or Resident Alien and can establish their identity by one of the above means.
- Children 15 years or younger renewing a US passport must submit both the passport and an original or certified copy of the birth certificate showing both parents’ full names.

- Adults 16 years of age or older: $212.05, payable by personal check or US Postal money order to the US Department of State.

Start by Gathering and Preparing Your Documents
- Fill out the online DS-11 application online, but DO NOT SIGN until directed to do so.
- Click “Create form” when finished.
- Ensure the printed form has two barcodes
Provide Proof of Citizenship
You need to supply one of the following:
- Previous U.S. Passport if you are over 15 years old
- Certified Birth Certificate from the state, county, or city
- Certificate of Citizenship or FS-240, DS-1350
- Certificate of Naturalization
Proof of Identity
Evidence of Immediate Travel
Supply 2 copies of one of the following:
- Travel ticket to a foreign country
- Travel itinerary or e-ticket with reservation confirmation
